• Request Restrictions: You may ask us not to use or share certain PHI for treatment, payment, or operations; however, we are not required to agree to all requests.

• Receive an Accounting of Disclosures: You can request a list of certain disclosures we have made of your PHI for purposes other than treatment, payment, and operations.

• Receive a Paper or Electronic Copy of This Notice: You may request a copy of this Notice at any time, even if you agreed to receive it electronically.

• File a Complaint: If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services (HHS). We will not retaliate against you for filing a complaint.
